Any or all of these steps may apply to you, depending on your individual situation.

  1. Furnish positive identification.
  2. Payment of applicable fees.
  3. Furnish proof of Utah residence.
  4. A photo.

Documents Needed to Obtain Utah ID Card

State Identification Card

  • State-certified birth certificate (No photocopies or miniatures)
  • Social security card (No copies will be accepted)
  • Verification of Utah resident address
    (May be verified by parent or guardian documentation for minor)
  • Second form of identification (see Acceptable Identification)

(State identification cards are valid for five years)

Positive Identification

When you first apply for a Utah Identification Card, you must provide proof of name, date and place of birth, resident address, legal presence, (If applicable), and proof of your social security number.

The division considers a state certified birth certificate (a hospital issued certificate, noncertified copies or miniaturized copies, will not be accepted.) and social security card as best evidence of identification for licensing purposes. Other forms of identification are accepted only if the state certified birth certificate and social security card are not obtainable. The division has discretion to require any additional evidence to verify this important information. Items on the following list are documents that the division may accept, if the state certified birth certificate and/or the social security card cannot be obtained.

  • Passport
  • Military documents
  • Church records
  • Nonresident Alien Registration
  • Current driver license
  • Home mortgage or lease papers
  • Matricular Consular/I.D. (Utah only)
  • I-94 card or INS I-551 card
  • Utah State I.D. card
  • Police Pistol Permit with photo
  • Indian blood certificate
  • School I.D. with photo
  • Court of Record documents

$18.00 - An Identification card
$13.00 - An Identification card for person with a disability

Photo

After all requirements for a ID card have been successfully completed, a photograph will be taken and you will receive a temporary photo ID card on the same day. The completed ID card will then be mailed to you in approximately three weeks.
